Appointments and Paperwork

Option 1, Everything Online:

1. Need a full eye exam or it has been over a year since your last exam
2. Have already made your appointment over the phone

You have 2 options:

1. If you have a previously saved form showing to the right, click to edit it and save time ->
2. or (first time here or good for entering multiple family members)

Option 2, Appointment Only - paperwork not needed:

1. Only wish to request an appointment and wish to fill out your forms when you get to the office
2. You only need to request an appointment for a follow-up exam for an existing problem

Option 3, Download and Print:

1. You would rather print your forms at home and bring them to the office. You still need to call and make an appointment however.
