Appointments and Paperwork

Option 1, Everything Online:

Use this option when:

• Need a full eye exam or it has been over a year since your last exam
• Have already made your appointment over the phone

1. Returning patients should log in and check to see if you  have a saved form
2. New patients (or other family members)   

Families only need one account! Just click on the "Fill Out New Form" button for each family member.

Option 2, Appointment Only - paperwork not needed:

Use this option when:

1. Only wish to request an appointment and wish to fill out your forms when you get to the office
2. You only need to request an appointment for a follow-up exam for an existing problem

Option 3, Download and Print:

Use this option when:

1. You would rather print your forms at home and bring them to the office. You still need to call and make an appointment however.
